Laser therapy is a recognized procedure for the surgical treatment of severe eye diseases. It is the only effective therapeutic procedure, especially for wet macular degeneration and retinal detachment caused by diabetes mellitus.

Laser therapy can also produce good results for other eye diseases. These include the so-called night cataract as well as the effects of thrombosis and bleeding. In our practice, various laser therapies are available to you.

Laser for glaucoma

There are various ways to treat glaucoma with laser. Here we present the ones used in the Sanoculus surgical centers.

The SLT laser is suitable as an alternative to treatment with eye drops. The SLT laser stimulates the outflow of aqueous humor and thus lowers intraocular pressure. This treatment is low-risk and can be carried out on an outpatient basis without much effort. Often, the intraocular pressure is lowered so well by the treatment that eye drops are no longer necessary. This treatment can also be repeated if the result is not yet sufficient or if the intraocular pressure rises again after a few years.

In CPC, the cells are treated by the ciliary body. The aqueous humor is formed in the ciliary body, but after treatment with the CPC laser, less of it is formed and the intraocular pressure decreases. This laser is often used when intraocular pressure can no longer be regulated with eye drops or when they are poorly tolerated. It is performed on an outpatient basis under light anesthesia.

In special forms of glaucoma (narrow-angle glaucoma), it may be necessary to open a small gap in the iris with the YAG laser. (actually Nd:YAG laser for neodymium-doped yttrium-aluminum-garnet laser).

Laser for secondary cataracts

The posterior cataract is a clouding of the posterior lens capsule of the natural lens of the eye after cataract surgery has already been performed. A cataract can occur shortly after the operation or many years later. For the patient, the vision becomes cloudier, similar to what it was initially before cataract surgery.

Since the lens capsule is very thin, the posterior cataract can usually be removed on an outpatient basis by laser surgery without any problems . The YAG laser (actually Nd:YAG laser for neodymium-doped yttrium-aluminum-garnet laser) is used to “shoot” a larger hole in the back capsule with several small holes. This allows the light to enter the eye again unhindered, and the cloudiness has disappeared.

Laser for retinal diseases

The retina lies inside the eye like wallpaper. Here, treatments can be carried out with the argon lasers . These treatments create a small local scar at the site of the laser. Indications are treatment of retinal holes and treatment of the retina in diabetic retinal changes. Our eye surgery centers at Elsterplatz & in Spandau have all common eye laser systems for the treatment of retinal defects, retinal diseases in diabetes or vascular occlusions, cataracts and glaucoma. Laser therapy is a completely painless treatment. In exceptional cases, we can give local anesthesia.

Frequently asked questions about laser therapy

In cataract surgery, the natural, clouded lens is removed and then an artificial lens is inserted. As a rule, the posterior capsule of the lens is left in place during the operation and then an artificial lens is inserted in front of it. This can lead to new problems. At the posterior capsule, new fibrous structures can form in the visual axis (capsular fibrosis). Or there are still lens residues inside the capsule, which become cloudy further in the course of the capsule. Every now and then, remaining cells have a tendency to multiply in order to rebuild parts of the lens (regenerative cataracts). These clouds can be gently removed with our laser therapy.

At the beginning, the eye is locally anesthetized by drops. The patient sits upright with his chin propped up at a “laser slit lamp”. This casts short-term laser beams on the eye. In this way, the cataract (or the clouded capsule) is separated in the middle. A gap forms through which sharp vision is immediately possible again.

Laser therapy is completely painless. At the beginning, our patients receive drops to numb the surface of the eye so that they do not feel any pain. The drops for pupil dilation/constriction (depending on the treatment) are also not noticeable. For this reason, you should be careful not to rub your eye about 30 minutes after the laser treatment, as this could still be numbed.

In the case of eye disease of the retina caused by diabetes mellitus , we can stop the progressive deterioration of vision with laser therapy. The treatment is adapted to the clinical picture. For example, the laser can be used to scar the retina over a large area in order to improve the oxygen supply to the macula. A macular edema that has developed can be lasered directly to prevent new vascular formation. In this way, we have a good chance of maintaining vision permanently.

With the tissue-sparing, subliminal laser, laser treatment of the macula is possible while avoiding thermal scarring. With the laser, we have micro-pulses for a very short time. The retina is not coagulated and the tissue remains intact. With the micro-pulse mode, we can treat many eye diseases such as diabetic macular edema (DME) or retinitis centralis serosa (RCS) in laser therapy.
